Premier Caregiver Services Team

John O’Callaghan

Owner/CEO

John developed an appreciation for all aspects of seniors aging when he was a young teenager and his great-grandmother from Grayson, Kentucky came to live with his family. Mama Yates lived to be 101 years old and was an integral part of John’s life, sparking his passion for senior care as a career.

After spending a large portion of his career in the business world, John left his corporate position at the Papa John’s company in 2007 and started down the path that would eventually lead to the founding of Premier Caregiver Services. In 2015, John became the Owner and Agency Manager of the caregiving company and now solely focuses on helping his team provide the best caregivers in the market.

In his spare time, John loves to boat, water ski, mountain bike, and spend time with his wife, Heather, their two boys, and one daughter.

Michael Carr

HR Manager

Michael Carr was born and raised in Louisville, Kentucky. He graduated from Trinity High School in 2013 and went on to further his education at Sullivan University where he graduated with a degree in business. Michael interned and then worked part-time with Dell Technologies before joining the Premier Caregiver Services team full time in 2019—a decision he considers one of the best he’s made in his life.

Michael was fortunate enough to grow up with four grandparents who all lived within 5 miles of his parent’s house. He was very close to each of them and spent a lot of his time at their houses, helping any way he could. Michael found his passion for helping the elderly, the injured, and the sick early, but his investment goes way beyond helping his own family, and he often tries to give back to the Louisville community in any way that he can including helping the city’s elderly and sick stay as healthy and comfortable as possible in their own homes.

When Michael isn’t working, he can be found watching his favorite sports teams play, spending time with family and friends, traveling with his wife, or playing with his 3 dogs—Fenway, Murphy, and Libby.

Jen Hull

Director of Marketing

If there’s a road map for navigating care through the stormy seas of dementia and Alzheimer’s—Jen probably helped draw it because she has been there!

With over two decades in healthcare marketing and business development, Jen brings equal parts strategy, heart, and humor to Premier Caregiver Services Team. Her career began not in a boardroom, but around her own kitchen table—helping her family find compassionate care for loved ones facing memory loss. That experience lit a fire that’s never gone out.

Jen’s not your average advocate. She’s part detective, part cheerleader, and part walking Rolodex—connecting families with the right support, smoothing the bumps, and making sure no one feels alone in the process. Whether it’s explaining long-term care plans or finding just the right caregiver match, Jen makes it feel easy (even when it’s not).

Originally from beautiful East Texas and now loving life in Louisville with her love, she blends Southern charm with no-nonsense clarity. She’s also known for her signature style, obsession with dog, and the ability to spot a good heart from a mile away.

Her motto?

“Dementia doesn’t mean dignity has to go out the window.”

She’s here to help families preserve both—with grace, grit, and a whole lot of compassion.

Sam Nation, LPN, CDP

Director of Client Care Services
Certified Dementia Practitioner

A Louisville native who grew up in Goshen, Kentucky, Sam found her passion for caregiving through deeply personal experiences. Caring for her Mimi, who had Alzheimer’s, and her younger sister with Cerebral Palsy inspired her to pursue a career in healthcare. With nursing studies at Sullivan University and experience in skilled post-acute care, long-term care, and personal care, Sam believes that heart and companionship make all the difference in dementia care.

At home, Sam juggles life with her three kids—a spirited oldest daughter and lively boy/girl twins—and two oversized dogs, a Great Pyrenees, and a German Shepherd. When she’s not hiking, reading, or enjoying the outdoors with her family, she’s focused on bringing joy and comfort to others, one client at a time.

Jennie Adams, CDP

Scheduler
Certified Dementia Practitioner

Having lived in a variety of places since moving from her hometown in California, Jennie Adams has been a resident of Illinois, Wisconsin, Arizona, and now Kentucky as a caregiver. She has over 20 years of experience working directly with individuals who have all types of dementia and disabilities as well as experience as a Licensed Assisted Living Manager since 2013. Jennie joined the Premier Caregiver Services team as a caregiver in 2022, but shifted into the Care Manager role in 2023.

In her spare time, Jennie enjoys gardening and spending time with her family which includes her husband of 11 years, Steven, their one son, and their many pets.

Loretta Moreno

Operations Manager

Originally from California, Loretta has lived in Kentucky for about 30 years, but spent the majority of her life in the Elizabethtown-area and only moved to Louisville a few years ago in order to be closer to her three grandchildren. Loretta graduated from West Texas State University in Canyon, TX, with a degree in business and worked in the restaurant industry for about 35 years. She spent a significant part of her career working with Dairy Queen’s corporate offices, where she served as the Vice President of Operations for 12 years.

Loretta’s familiarity with Premier Caregiver Services originates from her personal life—while looking for help caring for an elderly friend who had suffered a stroke, she met with a variety of home care agencies in the Louisville-area. After meeting with Premier Caregiver Services’ President, John O’Callaghan, she decided to hire Premier to provide home care services and, soon after, joined the team. Since joining Premier Caregiver Services, Loretta has become certified as an Alzheimer’s Association Community Educator.

In her free time, Loretta enjoys spending time with her family, especially watching her grandchildren participate in a variety of sports. She also loves boating, jet skiing, and traveling with her husband.

Sharon Bland

Caregiver Training Specialist

Sharon bland is a dedicated retired RN with a passion for compassionate caregiving. With a diploma from Saint Joseph Infirmary and a BSN from McKendree College, her extensive nursing journey is marked by her certifications in critical care (CCRN), hospice (CHPN), nursing administration (CNA-BC), and more. She is also certified as an Alzheimer’s Association Community Educator.

Beyond her medical expertise, Sharon is a devoted animal lover, finding joy in the company of dogs and cats. Her interests extend to the beach, gardening, reading, and even the occasional trip to the casino. As the heart and soul behind dementia and Alzheimer caregiving services training program, she not only offers invaluable support but also imparts her wisdom for our caregivers and new employees with the most up to date information.

Erica McCullough

Office Manager

Erica grew up in Southern Indiana, where she learned the value of hard work, teamwork, and perseverance through her passion for softball. She played throughout her childhood and continued competing at the college level. These experiences instill in her the values of commitment, organization, and leadership- qualities she now brings to her role at Premier.

A recent graduate, Erica is excited to start her professional journey with Premier Caregivers Services, supporting both the team and the families they serve. She enjoys building connections and helping ensure the office runs smoothly, so caregivers can focus on providing the highest level of care.

Outside of work, Erica enjoys spending quality time with her family and friends, especially at the lake, where she loves the chance to relax, recharge, and create lasting memories. Her strong roots in family and community fuel her commitment to caring for others with patience, compassion, and respect.

Cathy Pyrek

Hiring Manager

Cathy grew up in El Paso, Texas, where she earned her teaching degree at UTEP, and became an elementary school teacher. After getting married and moving to Colorado, she earned an MBA at the University of Colorado, which has shaped her understanding of organizational dynamics and leadership. A brief stint at the University of Texas at Austin got derailed and rerouted to Kent State University, where she earned an MA in Anthropology. Kent State is also where she taught Anthropology.

Though teaching has always been a cornerstone of her career, Cathy found her jam in administration. Over the years, she has spent a significant portion of her professional life working behind the scenes, taming bureaucracies so that they don’t drive folks crazy.

Cathy came to Premier Caregiver Services by way of Loretta Moreno. Their friendship had previously led them to volunteer regularly at facilities for the aging. Loretta also enlisted Cathy to assist with the care of a long-time friend in need of 24/7 home care.

Outside of work, Cathy is a person of varied interests and surprising expertise. She’s an unexpected know-it-all of dumpsters—yes, a perennial favorite topic at parties. Also of non-instrument navigation, just in case she’s ever lost at sea, which could happen because she’s also a sailor. She enjoys a lifelong passion for the violin, playing on a violin made by her great-grandfather. And she is an ardent lover of dogs, whose companionship adds joy to every day.
